What To Do Before Your First Pediatric Visit?

We are excited to welcome your family to Fairfield Pediatrics. Our goal is to make your first visit as smooth and stress-free as possible. To help us provide the best care for your child, please review the information below before your appointment.

STEP 1

Schedule Your Appointment

All visits at Fairfield Pediatrics are by appointment only.

When scheduling your child’s first visit, please let us know if: You are a new patient, Your child is a newborn, You are transferring from another pediatric practice, You have specific concerns you would like addressed.

STEP 2

Complete Your Forms Before Your Visit

To reduce wait times and streamline your check-in experience, we encourage families to complete all required forms before arriving.

New patient forms may include: New Patient Registration Form, Consent for Treatment, HIPAA Acknowledgment Form, Insurance & Financial Responsibility Form, Medical Records Release Form (if transferring care).

Completed forms may be submitted in advance or brought with you to your appointment. If you are currently at this stage click here to check out our forms.

STEP 3

What to Bring

Please bring the following items to every visit:

  1. Parent or guardian photo identification
  2. Current insurance card
  3. Immunization records (if transferring from another practice)
  4. Any medical records not previously sent to our office
  5. A list of current medications
  6. Any completed patient forms

Having these documents available helps us provide efficient and accurate care.

STEP 4

Arrive Early

Please plan to arrive at least 10 minutes before your scheduled appointment.

This allows time to:

Verify insurance information

Review paperwork

Complete any remaining forms

Update patient information if needed

Patients arriving more than 10 minutes late may be asked to reschedule depending on provider availability.

Transferring From Another Pediatrician?

If your child is transferring care from another practice, please request that medical records be sent to Fairfield Pediatrics before your appointment whenever possible. This allows our team to review important health history, immunization records, and previous treatments prior to your visit.
