Access to the patient portal is provided through MyCorewellHealthChart. You will receive an activation code during your visit or by email, which allows you to create your account and begin using the system.
Parents and guardians may request proxy access for children under 18 years of age. Limited access is available for patients aged 18 and older, in accordance with healthcare privacy guidelines.
