School & Sports Physicals

Helping Your Child Stay Active, Healthy, and Ready for School

What Is a School or Sports Physical?

A school or sports physical is a routine examination that helps determine whether a child is medically fit to attend school or participate in sports activities.

During this visit, we assess your child’s general health, review their medical history, and evaluate any conditions that may affect their ability to safely engage in physical activity.

This exam is not the same as a sick visit or problem-focused appointment. It is designed specifically for prevention and clearance purposes.

What to Expect During the Visit

During your child’s physical exam, we will review their medical history and immunization records, check vital signs such as height, weight, blood pressure, and heart rate, and perform a full physical examination.

We will also evaluate vision and hearing when appropriate and assess posture, joints, and musculoskeletal health, especially for children participating in sports.

If your child has a history of asthma, heart conditions, injuries, or other medical concerns, we will carefully review how these may impact physical activity and provide guidance where needed.

What You Should Bring

To ensure a smooth visit, please bring any required school or sports forms, download and fill the require Fairfield pediatrics form here for sports which contains your child’s immunization records if not already on file, a list of any medications your child is currently taking, and any completed paperwork from your school or sports organization.

Having these documents available helps us complete forms accurately and avoid delays.

Insurance and Payment

School and sports physicals may not always be fully covered by insurance plans. In some cases, they may be considered a separate service from a well-child visit.

Our team will help you understand any potential costs before your visit so there are no surprises.
